Self-Determination Independent Facilitation Support
SDP is another way to receive services from Regional Centers. It allows RC clients to have more freedom of choice in how they spend their RC budget funds.
As Independent Facilitators we help my client’s transition to SDP. This service is funded by your local Regional Center and is free to you as a Regional client.
Person-Centered Planning & Report Writing
Person-Centered planning creates a safe space in which participants can discuss their future dreams and visions. It includes them in the decision-making processes. For this meeting all supporters are invited and we meet in-office or virtually on Zoom.
Our client’s Person-Centered Plans include all of the wonderful information, details, stories, goals, dreams, resources, ideas, proposals, and supports that were shared during the PCP Meeting.
The final PCP report will be used by the Regional Center to help determine a clients budget.
Budget & Spending Plan Creation
The Spending Plan specifies how budget funds will be used to purchase services and supports for each fiscal year an individual is enrolled in the Self-Determination Program.
Life Leap Services helps client’s create a Spending Plan based off their Regional Center Budget. We also help clients to identify resources and services to include in the Spending Plan.
Advocating for YOU, or your loved ones, needs
Life Leap Services supports clients by attending the Individual Program Plan (IPP) meeting, budget meetings and Spending Plan Meetings with the Regional Center.
We help clients to identify, and advocate, for unmet needs and find services to support those needs.
Continued IF Support
We offer continued Independent Facilitation support throughout the SDP journey that goes beyond the initial transition support.
For on-going SDP clients we assist with navigating the Financial Management Services (FMS), hiring staff, and many levels of administrative support. For yearly renewals we will update the Person Centered Plan (if necessary) and create the Spending Plan for the new year.
Having an Independent Facilitator that take over the work of these tasks is a great relief. It allows you to benefit from the Self-Determination Program without having to worry about the administrative details.
Scholarships & Seeking Other Resources
We support clients beyond SDP by informing of them of all resources and supports that they may not be aware of to assist them. You can think of it like having a cheerleader on your side that wants you, and your family, to succeed.
